Sounds like the front bearing/retainer is worn, which would allow a lot of play in both the O/D unit, and the 4 speed part. Once the retainer wears, it also will allow a lot of free play in the direct clutch-pack, which would explain the slippage in rev.

Also check the seal between the booster and the master cylinder. They deteriorate over time, and create the same problems. The seal isn't available separately, but I have found that the seals for the headlight actuators fit perfectly with a little modification, and work like a champ.
