Smog Decal ?

Had my bi-annual smog test today. I passed, but almost failed on a technicality. There is no decal in the engine compartment detailing the smog equipment for the LT 5 engine. Is there supposed to be one, if so where can I get one? Anyone have a picture of it? Part #?

There 5s one on the cooling pack. You should be able to get it from GM. George
