Need some help with Vacuum and a Bolt.

I need some help with a misfire. I am pretty sure its a vacuum leak since I have tried everything else. Does some one have a diagram of the vacuum hoses for a 96 LT1?

Also I found out that one of my header bolts fell out. Its the one closest to the passenger side. I need to know the diameter and the length please.
