P0157 - Have code, but what's the remedy?

I went a bought a OBD II code checker which I thought was a pretty smart thing up until I realized it doesn't really tell me much. I have a P0157, 02 Sensor Circuit Low Volts (Bank 2, Sensor2). I'm going to assume it's an 02 sensor before or after the cat, but how would I know. Is there a guide of some kind that would give you more then the code and fault description?

Basically, what should I do to get rid of the code? I've cleared it and it returns almost every time I drive the car.

Bank 2 Sensor 2 = right rear (behind cat). Cause could be anything that causes low voltage i.e sensor pigtail or signal wires contacting ground, lean conditon from exhaust leak or other upstream cause, bad O2 sensor. As far as clearing the codes, that will take care of itself once diagnostic runs and does not fail. History will clear with time once the problem is corrected. Or you can clear with a scan tool or battery disconnect if you can't wait.

Also, Fuel Contamination will causes lean exhaust also and Lean injectors will cause it. Low fuel pressure is another culprit not to mention Vacuum or crankcase leaks. History clears after 40 consecutive warm up cycles if no error is detected. I had this problem not too long ago. If the O2 sensor remains below 39mV for more than 38 seconds it will set the code. If you have allot of milage and the original O2's, you might want to change them.
 
Actually, it's P0151, Bank 2 Sensor 1 and now I also get P0154, Sensor no activity on the same one. I suspect it's a bad 02 sensor but which one is this one? Left behind cat?

Rick, gotta go right now but here's a starter for you:

Bank 1 = Left side.
Bank 2 = Right side.
Sensor 1 = prior to cat.
Sensor 2 = behind cat.

P0151 same conditions as above as is P0154 likely bad or lazy O2.

How many miles?

Rick, If you still have the original O2's you might want to replace them. You might also have a bad TPS. Throttle Position Sensor. A bad TPS will keep your Computer guessing as to what it should be applying. If anyone else has any ideas, please speak up.
