C4C5 Please help

I have a clunking sound in the rack and pinion when you move the steering wheel rapidly from side to side. It sounds like a hammer tapping on the housing. I have also noticed what I think is a bit of shake in the front end. The ball joints look fine and the outer tie-rod ends are good. Are there inner tie-rod ends that can be replaced, or do I change the whole rack?

There are inner tie rods which can be replaced, most people just do the whole rack, as they are prone to wear and tear.
 
Is it a difficult job? Can it be done without removing the rack?
