Serpentine Belt

Any thoughts on why the Serpentine Belt might come off without breaking?

Have purchased a new belt to install but wonder if I might be working the symptom instead of the cure.

Is there anything special I should be aware of in changing the belt?

Had the same problem. Lost the belt without breaking. After I put the belt back on I noticed the belt tensioner was bent (sprung) about a quarter of an inch and the belt was not straight. I replaced both the belt and the tensioner.

That is what came to mind for me, too.
 
Something is out of alinement to make it ride off. If it is not the belt tensioner then you might be getting a worn bearing somewhere else, try moving the pullies to make sure you do not have any play in them or a bent shaft.
 
Yep. Turned out to be the idler pulley.
