Get two flat bladed screw drivers and stick them in on each side of the button on top of the knob and pry it out (be careful) when it is out you should see a small metal shaft inside with a c-clip on it (close all of the windows/doors/harch incase the clip goes flying). Remove the clip and pull the rest of the knob off (you may have to use some force).

You can start with a pry bar and a sledge hammer. First crash an opening on the roof with the sledge hammer and stand on the shifter. Ask two people to pry the shifter and if it does not come out kick the *sh_t* out of it...
