Which clutch parts to replace?

While I have the trans out I feel I should replace all the wear parts in the clutch - but which parts? Can someone give me a list of what components wear and therefore should be changed, before I put the tranny back?
 
Let the Games Begin.!! Clutch pressure plate, clutch plate (disc), throwout bearing ,pilot bearing or bushing in end of crankshaft. Resurface pressure plate if still salvageable and only reinstall with new bolts and proper locktite . And don't forget to lubricate the pivot ball for the clutch fork ( the thing that holds the throwout bearing).
 
Be sure and have the flywheel resurfaced (I think that's what Curtis meant when he said "resurface pressure plate"). :)
