Need Replacement 1976 Firestone Warranty Card

Major screw up caused the death of my original today....Can anyone help with a replacement? I would greatly appreciate it!

If the warranty card is the same for the 1977 Firestone recalled tires I can scan it for you front and back to the proper size when printed out.
It is an odd size paper measurement of 8.5” width and 14.5” length.
