New Horrible Rattle with clutch pedal down

As I was coming home last night I had a horrible rattle (preverbal "bolts shaking in a can" sound) with the clutch pedal depressed to the floor.

The clutch was replaced a year ago (10k) right before I bought the car. I recently replaced the master and slave (slave twice) but never had this noise.

Is it the Dual Mass Flywheel going bad? Loose fork? Throwout bearing? Did something come loose?

I can only assume that when they replaced the clutch , they probably resurfaced the flywheel, if they had replaced it with a SMF it should be making a ton of noise all this time (and I thought the noise from the SMF was in neutral with clutch pedal up?)
